New Patient Registration

Patients must live within the practice boundary to be eligible for registration at the practice. If you are unsure if your home address is within our boundary you can use the map below or please contact us for confirmation. We are unable to keep patients on our list who move out of this practice area.

How to Register

You will need to complete a registration form to register at the surgery. Please do this by collecting a form from the surgery and handing this back in with proof of who you are such as a passport or driving license and also proof of address such as a utility bill. You can also register using the online form.

Initially we will ask you to complete a health questionnaire as it is important that we have as much information as possible on your past medical history. We will, of course, be asking the appropriate authority to send your medical records once they have been retrieved from your previous General Practitioner, but this will take at least 2-6 weeks. You will be offered a new patient health check with the practice nurse.

We work as a Partnership and as such, GP’s do not have individual patient lists that they see alone, although every patient has a ‘Named Accountable GP’. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.
